Abstract

Le bégaiement est un trouble relativement fréquent. Il touche, à des degrés divers, environ 1% des adultes. De nombreuses personnes célèbres en ont souffert comme Démosthène, Lewis Carroll ou encore le roi Georges VI. Si certains individus n'en souffrent que de manière transitoire durant l'enfance, il représente un handicap permanent pour de nombreux autres. Ses conséquences psychologiques et sociales peuvent être importantes. Malgré son degré de prévalence et son impact sur la vie des individus, le bégaiement a fait l'objet d'assez peu d'études scientifiques et les publications à son sujet sont peu nombreuses. Des traitements ont été mis au point avec des résultats divers, mais les causes profondes du bégaiement et de son éventuelle guérison restent largement méconnues. De ce point de vue, le présent ouvrage comble un vide. Il apporte une première synthèse en langue française des connaissances scientifiques actuelles sur la question du bégaiement. Il en couvre toutes les facettes, depuis sa sémiologie jusqu'à son traitement, en passant par ses bases neurologiques et son impact linguistique et social. Il intéressera tant le praticien soucieux de comprendre et d'aider la personne bègue, que le chercheur qui y trouvera matière à réflexion et, espérons-le, un stimulant à poursuivre des travaux sur le bégaiement.

Abstract

Psychoanalysis as a long term modality is inaccessible to the average South African. In this book the authors describe how psychoanalytically orientated or psychodynamic psychotherapy can be practiced as a short-term endeavour and applied to contemporary issues facing the country. Psychodynamic work is currently undertaken by clinical psychologists, therapists, clinicians, trainers, teachers, clinical supervisors, consultants and researchers working in university settings, state hospitals, community projects, private practice and research. The debates, clinical issues, therapeutic practice and nature of research covered in the book are widely representative of the work being done in the country. The need for shorter term therapy models and evidence-based interventions is as acute in global practice as it is locally. The lessons learned in South Africa have broader implications for international practitioners, and the authors stress the potential inherent in psychoanalytic theory and technique to tackle the complex problems faced in all places and settings characterised by increasing globalisation and dislocation. The book is structured in three main sections. Psychodynamic Psychotherapy in South Africa is aimed at local and international practitioners and students, while non-specialist readers will find the text informative and accessible.

«L'objectif que je poursuis est de démontrer que lorsque l'attachement avec la mère n'a pas eu lieu, les enfants vivent une carence affective et sont troublés par la peur inconsciente, mais permanente, d'être abandon- nés. Les rapports harmonieux avec l'objet narcissique premier qu'est la mère constituent la base même du sentiment de sécurité affective, alors qu'au contraire, des rapports troubles à un moment critique constituent la base du sentiment d'abandon et des réactions d'insécurité, de tristesse et d'agressivité qui y sont inextricablement liées. Sensible aux difficultés d'adaptation et d'apprentissage des enfants inca- pables de se conformer aux attentes des adultes en autorité, j'ai résolu- ment décidé d'examiner les empreintes laissées par leurs expériences de séparations. Pour le petit enfant, exister et grandir ne sera bienfaisant et joyeux que s'il se sent en sécurité physique et en communication affective avec sa mère. Ce qui a lieu ou n'a pas lieu dans le temps de la relation symbiotique et des désirs narcissiques peut empêcher certains enfants de réaliser un véritable attachement. ».

Abstract

"Adolescents with behavior problems go by various labels, such as difficult-to-treat adolescents, juvenile delinquents, at-risk youth, violent youth, and youthful offenders. They may have disruptive behavior disorder or substance abuse disorder, and they may be involved with the criminal justice system. These youth have problematic behaviors, emotions, and ways of thinking that often affect not only their families, but also their community. These youth, their families, and their successful treatment represent the focus of this book. Although family members are often dissatisfied with the youth's behavior and intensely focused on the need for him or her to change, the youth rarely self-refers and often seems undisturbed by his or her own behaviors and is typically unmotivated for treatment. At the same time, parents and other family members seem heavily invested in viewing problems in the family as resulting from the adolescent's bad behavior. Therefore, some family members also are unmotivated or unwilling to be involved in the youth's treatment. Given the multifaceted barriers to adaptive change, treatment is needed that addresses the entire family system in a holistic way. Over the past 40 years, numerous researchers and clinicians have developed, tested, and refined Functional Family Therapy (FFT)--a short-term, structured, intensive family intervention model for delinquent and substance-using adolescents. FFT is a strength-based model with a focus on those risk and protective factors that impact the adolescent and his or her environment. On average, 12 sessions are conducted over a 3- to 4-month period. These sessions typically occur in clinics and home settings but can also be conducted in schools, mental health agencies, child welfare agencies, probation and parole offices, and aftercare systems. We have written this book with the clinician in mind. We often maintain a quite personal tone because we want to share and talk with you rather than taking a more traditional academic tone by talking to you. Often we use examples of specific youth and families (with disguised identities, of course) so clinicians can experience the clinical reality of these youth and families. We describe not only the goals of each intervention strategy and technique but also the specific steps and variations that are involved and necessary for success. At the same time, because FFT also derives strength from our history of widespread dissemination and training, we also share information relevant to decision makers, program administrators, clinical supervisors, clinical researchers, and even legislatures who have the responsibility of how best to allocate funds to provide the most effective services. In the remainder of this Introduction, we discuss FFT's conceptualization of change as a developmental process and outline the five major treatment phases of FFT. Then we explain the organization of this book. As we describe the FFT phases and the techniques and therapist qualities involved in each, our strategy in this book is to move from a broad focus to a narrow focus, then back again to a broad focus."--Introduction.
